Meetings

The consortium meets four times per year to share information and connect with other parents and professionals.

Member action teams

The following member action teams are involved in developing and implementing goals supporting the consortium mission.

Political action:

  • Seek resources for hospital to home transitions
  • Educate our political leaders on the requirements of children with complex medical needs.
  • Explore and identify issues families and children with special needs face.
  • Focus on lobbying for changes in policy, legislation and budgets to promote well being.

Medical home:

  • Develop a family resource guide to help parents of a child with special health care needs coordinate care and organize information.
  • Promote family/professional partnership in the care of children with special health care needs to families and caregivers.

Community alliance:

  • Drive recruitment efforts for new consortium members.
  • Foster partnerships across organizations, encouraging participation in activities and projects.

Funding:

  • Define and support transition of care, such as hospital to home, school and to all aspects of adult life.
  • Seek funding for children with complex medical needs in Illinois.
