Position Summary:
Works
within a multidisciplinary team to perform clinical care, provide education,
conduct research, provide and support leadership activities. Promotes the advancement of the nursing
profession at CMH and in the community-at-large. The role is unique to the APN’s
specialty area and in accordance with individual credentialing
Essential Job Functions:

Performs expert clinical care, education,
consultation, research, leadership and mentorship.

Manages acute and chronic health problems through
comprehensive history and physical examination, ordering, interpreting and
performing laboratory and radiology tests.

Prescribes pharmacologic and non-pharmacologic
interventions, and performing other therapeutic or corrective measures.

Promotes the advancement of the nursing profession at
CMH and the community-at-large.

Collaborates with physician team to deliver
evidenced-based care and establish best practices in clinical
care.

Develops or individualizes teaching plans/protocols
to meet patient and family learning needs. Identifies patient preferences and
needs including motivation to learn.
Evaluates and documents changes to the plan in the Electronic Medical
Record.

Maintains and closes EMR encounters and sends follow
up letter to referring physician according to Ambulatory Service Standards.

Performs job functions adhering to service principles
with customer service focus of innovation, service excellence and teamwork to
provide the highest quality care and service to our patients, families,
donors, volunteers, co-workers and others.
FTE status: FT-80
Requirements
- Master’s degree or higher from an accredited school of
nursing appropriate for certification as a Nurse Practitioner, Clinical Nurse
Specialist, or Nurse Anesthetist.

Minimum of two (2) years of nursing experience
required, preferably in pediatrics.

Licensure in the State of Illinois as an APN is
required.

As delineated in clinical privileges through
credentialing with a collaborative agreement and protocols.

Basic Life Support certification required.
