Physician will serve CMMG in a professional capacity and will be expected to
perform duties commensurate with and required by such position. Actively
supports the Hospital’s/division’s Continuous Quality Improvement and Customer
Service goals through the modeling of appropriate behavior and by providing a
work environment conducive to attaining these goals.
1. Furnishing medical services to pediatric patients admitted to the hospital
facility(ies) listed in Schedule 1 of the CMMG Physician Employment Agreement
(the "Outreach Hospital(s)") in accordance with the scheduling and other
requirements set forth in that Agreement. Such medical services shall include
those required of a pediatric hospitalist charged with acting as attending
physician in the neonatal intensive care and other critical care units, treating
pediatric patients in the Outreach Hospital’s emergency department, overseeing
the care and progress of pediatric inpatients (including making hospital rounds)
and attending deliveries.
2. Physician agrees to perform any other duties appropriate under the
Outreach Program which are reasonably assigned by CMMG.
3. Render services for CMMG on the premises of the Outreach Hospital.
4. Perform his or her duties under the supervision and control of CMMG.
5. Take action which Physician deems, in his or her reasonable discretion, to
be consistent with appropriate medical practice and in the best interest of the
patient.
6. Exercise his or her independent medical judgment in a manner consistent
with approved methods and practices in Physician’s profession.
7. Keep and maintain appropriate medical, time, financial and/or other
records relating to all professional medical services rendered by him or her
under this Agreement in the manner specified by CMMG and as required by CMMG’s
policies and by law. Prepare and maintain, in connection with such services, all
necessary and appropriate reports, records and correspondence in connection with
the services performed, all of which documentation shall be the property of
CMMG.
8. Attend each Outreach Hospital’s medical staff meetings, including
pediatric medical staff meetings on a routine basis, emergency department
medical staff meetings on a quarterly basis or as required by the Outreach
Hospital, and obstetrics/gynecology medical staff meetings on a quarterly basis
or as required by the Outreach Hospital.
9. Attend and participate in bi-annual Outreach All Staff Site Meetings and
Billing & Coding Inservices as scheduled.
10. Meet with either emergency department attending physicians, referring
pediatricians or obstetrics/gynecology attending physicians every other month,
or as necessary, to facilitate communication regarding the medical services
Physician is providing to the patients at the Outreach Hospital and other
concerns or issues related to the Outreach Program.
11. Devote his or her full professional time, skill, energy and attention to
CMMG in the manner and degree agreed upon by CMMG. Physician agrees not to
engage in any other medical practice or related business, nor to engage in the
practice of Physician’s profession to any extent whatsoever, except pursuant to
this Agreement, without the prior written consent of CMMG and the Outreach
Hospital.
12. Obtain and maintain throughout the term of his or her employment:
a. A license, without restriction, to practice medicine in the jurisdiction
in which the Outreach Hospital(s) is located.
b. Full active medical staff membership with privileges, in good standing, at
both CMH and the Outreach Hospital(s).
c. Participation in the Medicaid program(s).
d. Participation, at the request of CMMG, in each of the managed care
arrangements offered by CMMG, CMH and any affiliated and related organizations
and participation in each managed care arrangement in which CMMG, CMH and any
affiliated or related organizations participate.
Physician is required to:
a. Comply with the policies, practices, and procedures established by CMMG,
including without limitation, all billing procedures and policies and shift
log-in and log-out policies and procedures, as amended.
b. Comply with the By-Laws of CMMG, the By-Laws of the Medical and Dental
Staff of CMH, and the By-Laws of the Children’s Memorial Hospital Faculty
Practice Plan, an Illinois not-for-profit corporation including any changes or
amendments thereto.
c. Comply with the policies and procedures of the Outreach Hospital(s) and
such hospital’s medical staff by-laws.
Requirements
1. Physician license in the State of Illinois and/or Indiana
2. Board certified or board eligible in pediatric specialty/sub-specialty.
Board eligible physicians must achieve full board certification within six (6)
years of appointment.
3. Demonstrated leadership, communication, and interpersonal skills.
4. PALS, ATLS or ACLS certified.
