What can we (parents) expect at the initial evaluation?
What information do we (parents) need to bring when we come for the first visit?
Please bring with you any medical records pertaining to your child's illness (laboratory test results, educational evaluations, neuropsychological evaluations, etc.). If your child has had any type of scans (MRI, CT, etc.), please bring a copy of the actual films (if possible) for the neurologist to review as well as a copy of the written report. If you are being referred by your pediatrician please, if possible, bring a statement from your doctor detailing his/her concerns and why he/she is referring your child to a neurologist. This is very beneficial as it allows the neurologist to pinpoint your pediatrician's concerns. To avoid delays in registration, please be sure you have your insurance card with you as well as a copy of any needed referral(s) from your primary care physician.
