If you would like a copy of your child's Gait or Upper Extremity Analysis report, please download a Release of Information form on the Health Information Management department homepage. This page has all the information you need to obtain your child's Motion Analysis Report. After downloading form, please specify "Motion Analysis Report" in the "type of information to be used or disclosed" section of the form.  Note: please contact the Motion Analysis Center for copies of Sports Biomechanics Program reports.
