How do I make an appointment?

Patients, parents or physicians should contact the Motion Analysis Center for scheduling or questions at 773.327.1936. The referring physicians should sign a prescription describing the test(s) to be performed (gait analysis, upper extremity analysis, foot pressure analysis, all, etc.). Please ask your physician to fax the most recent visit notes to our office at 773.327.1937.
