Applying to our program

Documents for you to submit

  1. Please complete and sign either the Common Child and Adolescent Psychiatry Residency Application form or the McGaw Medical Center of Northwestern University Graduate Medical Education application form. The completed application should be mailed directly to us.
  2. Please include an autobiographical statement if using the McGaw application form.
  3. Please include a current copy of your C.V. if using the McGaw application form.
  4. Please include photocopies of your original Step 1, 2, and 3 USMLE or COMLEX examination results, with dates.
  5. Please include a photocopy of your visa or citizenship papers if you are not a U.S. citizen by birth.
  6. Please include a copy of your ECFMG certificate if you are an international medical graduate.

 Documents for you to request from others

  1. Please ask your training director to complete the Attesting to General Psychiatry Eligibility form and mail it to us.
  2. Please ask your medical school to send us an official dean’s letter.
  3. Please ask your medical school to send us an official copy of your transcript.
  4. Please request a letter of recommendation from your training director. If you have been in more than one residency program, we will need a letter from each of your training directors.
  5. Please request letters of recommendation from at least two faculty members in addition to your training director(s).
