Preparing for your child's first appointment

  • Five days before the appointment, please stop any medications containing antihistamines that your child takes. Do not stop any other medications. A list of antihistamines is listed in the table below. Please check with your physician or pharmacist if you are unsure.
  • You may register online here or call 1.800.KIDS DOC.  Registration must be completed before your child can be seen in our clinic.
  • We strongly advise that you arrive 15 minutes prior to your appointment time. A parent or legal guardian must accompany the child to the appointment.
  • Allow yourself 2 to 3 hours for the first visit.
  • Please complete the attached questionnaire about your child's medical history and bring it with you to your first appointment.
  • Bring copies of your child's past allergy test results if applicable.
  • Please bring all necessary documents such as proof of insurance and a referral form.

The Allergy/Immunology program at Children's Memorial is a teaching program and your child will initially be seen by a medical student, resident, or fellow and then by an attending physician. However, if your appointment is with the APN (advanced practice nurse), you will be seen only by the APN and the attending physician.

If your child develops chicken pox, please cancel your appointment and schedule a new one.

  • If you fail to show for your appointment and do not cancel, you must call to reschedule.
  • If you need to cancel your appointment due to an illness or emergency, please contact us as soon as possible.
  • If you are more than 30 minutes late for appointment, your child may not be seen that day and the appointment will need to be rescheduled.
  • NO SHOW Policy: A patient who fails to show for the appointment or one who cancels within 24 hours (or one business day) of the appointment time is considered a “no-show.” If 3 or more no-shows occur within one year, future appointments will not be scheduled.
  • Co-pays will be collected at the time of service.
